Who should use birth control?

Sexually active women who don’t want to become pregnant should consider birth control. Timed pregnancies are important for several reasons; research continues to show that good preconception practices like weight loss, exercise, and nutrition affect the pregnancy and baby’s health. Using effective measures until you’re ready to have a child helps ensure that you are financially, emotionally, and physically ready to have a child. Even older women may need it if they don’t want a later-in-life pregnancy.

What kinds of birth control exist?

Birth control methods fall into four main groups: barrier methods, hormonal methods (including emergency contraception), long-acting reversible contraception (LARC) methods, and sterilization.

Barrier methods include male and female condoms, diaphragms, cervical sponges, spermicides, and cervical caps.
Birth control hormones exist as daily oral pills, weekly patches, monthly rings, and a twelve-week recurrent injection.
The CDC recommends LARCs, available with or without hormones, for younger women. One popular LARCs is Nexplanon , a small rod inserted in the arm containing progesterone and that lasts three years. Skin Cancer is the most common Cancer diagnosis in the United States.

Skin cancer is the most common cancer in the United States with more people diagnosed than all other cancers combined. The main types of skin cancer are squamous cell carcinoma, basal cell carcinoma, and melanoma. Melanoma is much less common than the other types but much more likely to invade nearby tissue and spread to other parts of the body.

Types of Skin Cancer

Skin cancer is a disease in which malignant (cancer) cells form in the tissues of the skin.

Basal cell carcinoma and squamous cell carcinoma of the skin, also called nonmelanoma skin cancer, are the most common forms of skin cancer. Most basal cell and squamous cell skin cancers can be cured.

Melanoma is more likely to spread to nearby tissues and other parts of the body and can be harder to cure. Melanoma is easier to cure if the tumor is found before it spreads to the dermis (inner layer of skin). Melanoma is less likely to cause death when it is found and treated early.

Symptoms of Skin Cancer

Basal Cell Carcinoma: Areas on the skin that look like scars, lumps or nodules that are shiny or have visible blood vessels; crusty, itchy patches of skin that may bleed or have a depression in the middle; or red patches of skin that look like eczema.

Squamous Cell Carcinoma: any sore that does not heal or comes back after healing; rough-feeling growths/bumps that get crusted over and bleed; patches of skin that are flat, scaly, and red; or white spots in the mouth, on the tongue, gums, or cheeks.

Melanoma: can present as moles, scaly patches of skin, open sores, raised bumps, or black marks under finger or toenails. Use the ABCDE memory device to spot the warning signs of melanoma on the skin: Asymmetry, Border, Color, Diameter, Evolution, or the Ugly Duckling method, where any mole or marking that does not look like the others on your body is worth getting checked by a dermatologist.

Screening for Skin Cancer

There is no official recommendation from the American Cancer Society for early detection of skin cancer, but most doctors will recommend checking your own skin once a month, especially if you are at a higher risk of developing skin cancer. What kinds of birth control exist?

Birth control methods fall into four main groups: barrier methods, hormonal methods (including emergency contraception), long-acting reversible contraception (LARC) methods, and sterilization.

Barrier methods include male and female condoms, diaphragms, cervical sponges, spermicides, and cervical caps.

Who is Gisela Paredes, DNP with Family First Family Medical Practice?

Gisela Paredes is a Family Nurse Practitioner specializing in providing true patient centered care, meaning her patient’s specific health needs and desired health outcomes are the driving force behind all health care decisions and quality measurements. Gisela practices a full spectrum family practice. She sees patients of all ages, including women’s health, adolescents, adult primary care, and geriatrics.

Gisela graduated from nursing school in 2016 and during her time working as a nurse in the hospital, she completed her Master of Science in Nursing degree at Chamberlain College of Nursing. In her pursuit to provide top quality care to the community she serves, Gisela recently obtained her Doctors in Nurse Practitioner degree (DNP). She has over fifteen years of healthcare industry experience working in the Surgery Department at Antelope Valley Hospital and thirteen years for a large non-profit hospital.

As daughter of Mexican immigrants, Gisela has first-hand experience working with the Latino community and is intimately aware of cultural, social, and behavioral norms of the community. 2) Family First Family Medical Practice has opened a same day clinic to allow expanded services to the community and offer more appointments. The goal is to staff a primary care from 8:00am to 8:00pm allowing for appointment after the day is done and give more access to healthcare. We also want patients to avoid going to the ER for simple acute problems like colds, flu, or pediatric ailments.

3) Family First Family Medical Practice hired Dr. Peter Zhang, MD to offer Sports Medicine to local athletes and weekend warriors. Given their background in musculoskeletal medicine, sports medicine physicians often help any patient suffering from certain conditions whether they are a serious athlete or not. They’re also excellent resources for individuals who are just beginning an exercise or training program.

Who should use birth control?

What kinds of birth control exist?

Birth control methods fall into four main groups: barrier methods, hormonal methods (including emergency contraception), long-acting reversible contraception (LARC) methods, and sterilization.

Barrier methods include male and female condoms, diaphragms, cervical sponges, spermicides, and cervical caps.
Birth control hormones exist as daily oral pills, weekly patches, monthly rings, and a twelve-week recurrent injection.
The CDC recommends LARCs, available with or without hormones, for younger women. One popular LARCs is Nexplanon , a small rod inserted in the arm containing progesterone and that lasts three years. Two others are Mirena and KYLEENA , which are seven and five year IUDs that also contain progesterone. There is also a copper IUD, Paragard , which lasts ten years.

Diabetes can over time potentially cause harm to your nerves especially if you do not control your blood sugar effectively. High blood sugar levels over many years can cause nerve damage. This damage is called Diabetic Peripheral Neuropathy and it can be painful.

Golfer's elbow (Lateral Epicondylitis) is a soreness or pain on the outside (lateral) side of the upper arm near the elbow. It also has alternative names such as Epitrochlear bursitis; Lateral epicondylitis; Epicondylitis - lateral; Tendonitis - elbow. It also can become a chronic issue caused by swelling of the tendons that connect the forearm to the outer side of the elbow.

The swelling ultimately damages the muscles and tendons from using them over and over again. That is why Golfer's elbow can be diagnosed as an overuse injury. Pain is often experienced on the outer side of the elbow, and the condition can be extremely painful. While playing tennis regularly can result in this condition, many patients treated for this condition don’t actually play tennis. Repetitive activities cause the tendons to get overworked, causing small tears.

To prevent this type of overuse injury, you simply have to not overuse it. If you start to have pain in the lateral side of the elbow (Golfer's Elbow), the best way to try to avoid the pain is to do things with the palm-up position. By putting your palm up, you transfer the forces from the hand to the medial side of the elbow.

Conservative, Nonsurgical Treatments

Patients with Golfer's Elbow are best treated non-operatively. Depending upon the nature and extent of the inflammation, there are many treatment options available which are successful in 80% to 90% of Golfer's Elbow cases.

RELAXATION. Stop the activities that are causing the pain. Your pain is telling you to rest the muscles by taking a timeout from using it. You will need to take a break from taking part in sports as well as refraining from heavyweight activities.
ANTI-INFLAMMATORY MEDICATIONS. Either over-the-counter (Ibuprofen and aspirin) or prescription anti-inflammatory medications can reduce swelling and pain.
EQUIPMENT INSPECTION. If you use a tennis racquet regularly, have it professionally inspected to ensure the size or weight of the racquet is appropriate.
TOPICAL COMPOUND CREAMS. Can be rubbed onto the affected area.
PHYSICAL THERAPY. Exercises that concentrate on reinforcing the forearm muscles are beneficial.
MUSCLE-STIMULATING PROCEDURES. Prescribed by your doctor and physical therapist improve the healing of the tendons. Ultrasound and massaging with ice also help.
BRACING. A brace can also ease symptoms.

Prolia® is a prescription medicine used to treat osteoporosis in women and men who are at high risk for fracture, meaning have had a fracture related to osteoporosis, or who have multiple risk factors for fracture or cannot use another osteoporosis medicine or other osteoporosis medicines did not work well. It is an injection every six months.

What is Osteoporosis?

Osteoporosis is a bone disease in which your bone density decreases, causing weak and brittle bones that are easily susceptible to fractures. Osteoporosis fractures commonly occur in your hips, wrists, or spine. Although osteoporosis can affect both men and women, it’s most prominent in post-menopausal women. Occurring more rapidly in women after menopause than prior to menopause, osteoporosis weakens your bones making them more susceptible to fractures. These fractures can impact your life in many ways and decrease the things you like to do. Medications and exercises can help slow your loss of bone mass and strengthen weak bones.

What are the symptoms of osteoporosis?

Common symptoms include:

Poor posture or a stooped posture.
Mid-back or low back pain
Bone fractures that happen easily
Loss of height over time

At first, you may not even be aware that your bone density is decreasing until you experience a fracture. Sometimes, osteoporosis symptoms go unnoticed and undetected for years; so if you are at higher risk of developing the condition, it’s a good idea to get a bone density test to determine your bone health.

Who is at risk for developing osteoporosis?

Caucasian and Asian women are in the highest risk group for developing osteoporosis. Also, your age, family history, and body composition can lead to an increased risk. Women and men who are thin and have a smaller bone structure are more prone to osteoporosis.

Taking certain medications may also weaken your bones, and adults with a history of hormone imbalances or thyroid disorders are at a higher risk. Poor diet, a sedentary lifestyle, and other medical conditions like rheumatoid arthritis can also put you at risk for developing osteoporosis.

What are my treatment options?

The goal of osteoporosis treatments is to prevent further bone density loss and to strengthen your bones to prevent fractures. Our team may prescribe medications that help protect your bones or hormone therapy to decrease bone loss. Our team uses ultrasound-guidance technology to administer injections like Prolia® that help reduce the effects of osteoporosis.

Additionally, making lifestyle changes such as quitting smoking or becoming more active can also help slow the progression of the disease and keep your bones strong. Taking supplements to increase your vitamin D and calcium levels may also help to control the symptoms of osteoporosis and strengthen your bones.

Understanding Prolia

Prolia® is a manmade antibody designed to bind to a protein receptor on the surface of bone cells known as osteoclasts. These cells are responsible for bone breakdown. Prolia® slows down osteoclast formation, activity, and survival, thereby reducing bone breakdown and promoting normal bone formation. Prolia® is an injectable drug administered under the skin every six months. It is generally very well tolerated, but may rarely cause low calcium levels, atypical fractures, and jaw problems.

What is a Bone Density Dexa Scan?

Bone density scanning, also called dual-energy x-ray absorptiometry (DXA) or bone densitometry, is an enhanced form of x-ray technology that is used to measure bone loss. DXA is today's established standard for measuring bone mineral density (BMD).

DXA is most often performed on the lower spine and hips. In children and some adults, the whole body is sometimes scanned. Peripheral devices that use x-ray or ultrasound are sometimes used to screen for low bone mass, mostly at the forearm

Why is a Bone Density Scan Recommended?

DXA is most often used to diagnose osteoporosis , a condition that often affects women after menopause but may also be found in men and rarely in children. Osteoporosis involves a gradual loss of bone, as well as structural changes, causing the bones to become thinner, more fragile and more likely to break.

DXA is also effective in tracking the effects of treatment for osteoporosis and other conditions that cause bone loss.

The DXA test can also assess an individual's risk for developing fractures. The risk of fracture is affected by age, body weight, history of prior fracture, family history of osteoporotic fractures and lifestyle issues such as cigarette smoking and excessive alcohol consumption. These factors are taken into consideration when deciding if a patient needs therapy.

Bone density testing is strongly recommended if you:

are a post-menopausal woman and not taking estrogen.
have a personal or maternal history of hip fracture or smoking.
are a post-menopausal woman who is tall (over 5 feet 7 inches) or thin (less than 125 pounds).
are a man with clinical conditions associated with bone loss, such as rheumatoid arthritis, chronic kidney or liver disease.
use medications that are known to cause bone loss, including corticosteroids such as Prednisone, various anti-seizure medications such as Dilantin and certain barbiturates, or high-dose thyroid replacement drugs.
have type 1 (formerly called juvenile or insulin-dependent) diabetes, liver disease, kidney disease or a family history of osteoporosis.
have high bone turnover, which shows up in the form of excessive collagen in urine samples.
have a thyroid condition, such as hyperthyroidism.
have a parathyroid condition, such as hyperparathyroidism.
have experienced a fracture after only mild trauma.
have had x-ray evidence of vertebral fracture or other signs of osteoporosis.

How is a DEXA Scan measured?

Your T-score refers to the amount of bone you have in comparison to an individual at peak health and bone density . Most often, young adults are the source of comparison here– just because they are most likely to be in optimal health. This doesn’t mean that you need to compare yourself to them in other ways– it’s just the standard for medical comparisons.

A Normal T-Score

…Is a value of -1 or higher . If your T-score is between -1.1 and -2.4, you will be diagnosed with osteopenia , a medical condition that refers to a patient having low bone density. Scores lower than -2.4 fall into the category of osteoporosis , or bone disease. The lower your T-score, the higher your risk for bone fractures.

Z-Score

Your Z-score, on the other hand, compares your results to other patients of the same size and gender within your age group. This number provides a comparative analysis of how you compare to your peers. This number can be helpful in determining what types of treatments you may be able to pursue, and the extent to which they may be effective. Furthermore, if your test results are extremely abnormal for your group, your doctor may want to look at root causes, as there may be an underlying complication or condition causing your bone mass to be so different .

A Normal Z-Score

Is within significant ranges for your gender, age, and size group. If you fall well outside the normal bounds for this group, your doctor will request more tests and help you find out what is amiss.

What Is A “Normal” Bone Density?

As mentioned above, a normal bone density rating is above -1.0. A number in this range indicates that your bones are a healthy density, having enough physical structure to protect themselves from unnatural physical damage.

Is there treatment for Osteoporosis?

Speak with your provider about treatment options available. Healthy living with proper nutrition is recommended, and based on individual assessments, weight bearing exercises may be recommended. There are several medications available and recommended based on your individual needs.

How does someone become a sports medicine doctor?

Generally speaking, there are two types of sports medicine doctors: primary care sports medicine doctors and orthopedic surgeons. Our primary care sports medicine doctors are fellowship trained in sports medicine with baseline training in primary care or emergency medicine.

How can I prevent injuries?

Athletes can prevent injuries by developing good warm-up habits, training properly, and taking good care of their bodies and equipment.

Specifically, athletes can: perform various warm ups and seek medical advice on ways to improve performance.

Take time to stretch: A warm-up that includes dynamic stretches—a cross between an exercise and a stretch—helps move and lengthen your muscles, preventing injuries and improving performance. Plan to warm up before both training and events.

Develop a training plan: It’s best to increase activity gradually, avoiding big jumps in intensity or duration. Incorporating rest days allows the body to heal, while diversifying your exercise routine helps avoid overuse injuries.

Listen to your body: Learn your limits and avoid overtraining. Frequent use of oral medications to cover up pain can slow healing. Use ibuprofen—or “vitamin I”—and other anti-inflammatories sparingly.

Replace old equipment: Soccer players and runners often suffer from shin splints, a painful condition caused by excessive training or running in worn-out shoes. To prevent this injury, shoes should be replaced after 300 to 500 miles of use. Also, be sure to replace any piece of equipment that no longer fits securely.

Eat a balanced diet and hydrate: What you eat and drink is just as important as your workout. Proper nutrition helps you fuel your athletic performance and recover from training. Plan and prepare healthy portable snacks or meals.

He received his medical degree from the University of Maryland, finished a Family Medicine Residency at the University of California, Riverside in Palm Springs where he was Chief Resident and completed a Sports Medicine Fellowship at the University of Texas, Rio Grande Valley. During his education and training he also served as a clinical and professional research assistant at Columbia University, New York and the University of Texas. His passion for learning also helped Dr. Zhang earn the prestigious Intern Resident of the Year while at UCR in Palm Springs. He is Board Certified through the American Board of Family Medicine.

His professional affiliations include The American Medical Society of Sports Medicine (AMSSM), the American Academy of Family Physicians (AAFP), and the Chinese American Medical Sociaty (CAMS). He is a Fellow of the American Academy of Family Physicians and holds a Certificate of Added Qualification in Sports Medicine.

We introduced Sublingual Immunotherapy last year and has seen tremendous results. Unlike injection immunotherapy, which is given as shots, sublingual immunotherapy is given as drops under the tongue. Sublingual Immunotherapy is a different approach with allergic disease as it targets with treatment the root cause, not just the symptoms. Once tolerance is built, it can be permanent for many patients and remove the need for monthly shots. It is very safe, for both adults and children. Patients take the drops in the convenience of their own homes instead of going to a doctor’s office every week for shots. The World Health Organization (WHO) has endorsed sublingual immunotherapy as a viable alternative to injection immunotherapy. Who should use birth control?

What kinds of birth control exist?

Birth control methods fall into four main groups: barrier methods, hormonal methods (including emergency contraception), long-acting reversible contraception (LARC) methods, and sterilization.

Barrier methods include male and female condoms, diaphragms, cervical sponges, spermicides, and cervical caps.
Birth control hormones exist as daily oral pills, weekly patches, monthly rings, and a twelve-week recurrent injection.
The CDC recommends LARCs, available with or without hormones, for younger women. One popular LARCs is Nexplanon , a small rod inserted in the arm containing progesterone and that lasts three years. Two others are Mirena and KYLEENA , which are seven and five year IUDs that also contain progesterone. There is also a copper IUD, Paragard , which lasts ten years.
Sterilization includes vasectomy and tubal ligation.

Why is a Bone Density Scan Recommended?

DXA is also effective in tracking the effects of treatment for osteoporosis and other conditions that cause bone loss.

Bone density testing is strongly recommended if you:

are a post-menopausal woman and not taking estrogen.
have a personal or maternal history of hip fracture or smoking.
are a post-menopausal woman who is tall (over 5 feet 7 inches) or thin (less than 125 pounds).
are a man with clinical conditions associated with bone loss, such as rheumatoid arthritis, chronic kidney or liver disease.
use medications that are known to cause bone loss, including corticosteroids such as Prednisone, various anti-seizure medications such as Dilantin and certain barbiturates, or high-dose thyroid replacement drugs.
